LEVELUP ATHLETE EXPO WAIVER & RELEASE OF LIABILITY

LevelUp Athlete Expo – Waiver, Release of Liability, and Media Consent

1. GROUP REGISTRATION ACKNOWLEDGMENT

The individual completing this registration (“Purchaser”) confirms that they are purchasing tickets on behalf of all listed participants.

By completing this purchase and agreeing to this waiver, the Purchaser affirms that:

• They are the parent or legal guardian of all listed minor participants, or
• They have obtained permission from the parent or legal guardian of each participant

The Purchaser agrees to all terms and conditions of this waiver on behalf of every participant included in the registration.

This agreement applies to all participants associated with the purchase.

2. ASSUMPTION OF RISK

I understand that participation in the LevelUp Athlete Expo (the “Event”) may include physical activity, movement-based instruction, and participation in an athletic training environment.

I acknowledge that gymnastics and related activities carry inherent risks, including but not limited to:

• Falls, slips, or collisions
• Muscle strains, sprains, or other injuries
• Equipment-related incidents
• General physical exertion

I voluntarily assume all risks associated with participation in the Event on behalf of all registered participants.

3. RELEASE OF LIABILITY

In consideration for being allowed to participate in the Event, I hereby release, waive, and discharge:

Salto Sports Group, its owners, employees, contractors, representatives, guest athletes, speakers, vendors, affiliates, and the host facility

from any and all liability, claims, demands, or causes of action arising out of or related to:

• Injury or illness
• Loss or damage to personal property
• Participation in any Event-related activity

This release applies to any claims caused by negligence or otherwise, to the fullest extent permitted by law.

4. MEDICAL CONSENT

I certify that all participants included in this registration are physically able to participate in Event activities.

In the event of an injury or medical emergency, I authorize Event staff to obtain emergency medical treatment if necessary.

I understand that I am responsible for any medical expenses incurred.

5. MEDIA RELEASE & CONTENT CONSENT

I understand that the LevelUp Athlete Expo is a content-driven event and may include:

• Photography
• Video recording
• Social media content creation

I grant permission to Salto Sports Group and its partners to:

• Capture photos and video of all participants included in this registration
• Use, reproduce, and distribute this content for marketing, promotional, and educational purposes
• Share content across social media, websites, and other digital or print platforms

I understand that no compensation will be provided for the use of this content.

This agreement applies to all participants included in the registration.

6. EVENT PARTICIPATION & EXPECTATIONS

I understand that:

• The Event includes breakout sessions, group activities, and interactive experiences
• Participation is voluntary and may be adjusted based on comfort level
• Athletes are expected to behave respectfully toward staff, speakers, vendors, and other participants

Salto Sports Group reserves the right to remove any participant for unsafe or inappropriate behavior. No refunds will be issued in such cases.

7. REFUND POLICY

All ticket purchases are final unless otherwise stated by Salto Sports Group.

In the event of unforeseen circumstances, Salto Sports Group reserves the right to modify event details, schedule, or programming.

8. AGREEMENT & ACKNOWLEDGMENT

By completing a purchase and selecting the waiver agreement at checkout, I acknowledge that:

• I have read and understand this waiver in full
• I voluntarily agree to all terms and conditions
• I am at least 18 years of age, or I am the parent/legal guardian of the participants included in this registration
• I am agreeing to this waiver on behalf of all listed participants
